How to add 5 Request approval actions in one workflow with infopath form?

Hello, I read the 'Nintex Workflow 2007 SDK 1.2' pdf document,  and in the section 1 -  InfoPath Forms Integration,  it show me that how to configure the approve & reject button rules for the 'Request approval action' in the workflow.

So I have a question for you, if I add 5 Request approval actions in one workflow, should I add 5 approve& reject buttons in the infopath form and add 5 rules for them as well? How can I add 5 rules for them?
