Workflow not starting automatically

I have associated a nintex 2007 workflow to infopath form library on SharePoint 2007 , Workflow has to send a email notification and update status field . The workflow is only working once or randomly , even if the notification is not send workflow column is updated as complete. Start up option is set to "When item is created" and all forms are submitted by users and not system account. Any help is appreciated
